Sample – Community Needs Assessment

Like many not-for-profit hospitals, as a member of the Adventist Health System, Florida Hospital Memorial Medical Center and Florida Hospital Oceanside, began its journey in developing a Community Health Needs Assessment and Community Health Plan alongside it’s sister hospitals across the state of Florida in 2012 in order to make improvechna-coverments in the health of the communities in which they serve.

The needs assessments, which are conducted every three years determine the major needs in the community. The data from the needs assessment is then used to develop a Community Health Plan with the goal of making health improvements through education, research, financial assistance, special clinical and non-clinical programs, and the provision of spiritual resources.

In order to be considered community benefit as part of the Community Health Plan, an activity must address a need documented in the assessment and have at least one of these objectives:

  • Improve access to health care services
  • Enhance the health of the community
  • Advance medical or health care knowledge
  • Relieve or reduce the burden of government
